Getting Started at Sensory KIDS
The following steps apply to families who are seeking occupational therapy services only. Families interested in speech and language therapy, counseling, or our home visit program should contact our office for further information on getting started.
STEP 1: Call our office at 503.575.9402 or email us directly at firstname.lastname@example.org
Our front staff will gather more information about the services you are looking for and begin the process by adding your family to our wait list.
STEP 2: Verify your family’s insurance eligibility and benefits
Once we have your insurance information on file, we will obtain a verification of your benefits and eligibility that will provide an estimate of coverage before services are rendered. If a PCP referral or authorization is required by your insurance plan, our office will work with your family to facilitate getting one.
STEP 3: Complete and return intake paperwork
Provided in the links at the bottom of this page is our intake paperwork. Our office will also send PDF versions of the paperwork via email upon your initial inquiry. Paperwork can be mailed out upon request.
The following forms will need to be completed and returned before an evaluation appointment can be offered:
- Intake Patient Form
- Short Sensory Profile (not available online – will be sent via email)
All other policy forms and releases will be reviewed and signed at your family’s evaluation appointment.
STEP 4: Scheduling an initial evaluation appointment and parent meeting
We will do our best to accommodate your family’s scheduling needs. Evaluations normally take place Monday through Wednesday mornings and may run 60 to 90 minutes. Evaluation appointments are scheduled at least a month in advance with the exception of cancellations that occur on our schedule. Written reports will be available 2-3 weeks after the evaluation appointment. Comprehensive written reports can be requested for an additional fee.
1-hour, adult-only parent meetings are scheduled at least a week after the evaluation appointment. Parent Meetings are covered by insurance and are used to discuss evaluation results, goals, and treatment plan with the evaluation therapist.
STEP 5: Scheduling treatment
We consider both your family’s availability and the availability of our therapists when scheduling treatment. Please expect a small wait following your evaluation appointment and parent meeting before treatment can be scheduled. The processing of insurance authorizations may extend this wait time further.
