Intake Details

How to Get Started

Each step in our intake process is detailed below. Patient Intake Forms for both children and adults can be filled out online. Links to these forms will be found at the bottom of this page.

STEP 1: Contact Us

Call our office at 503.575.9402, email us directly at info@sensorykidsot.com, or choose the “Inquire Today” bottom at the top-right hand of the page.

STEP 2: Complete and return the intake paperwork

We require the completed return of the Patient Intake Form, Parent Checklists, and Short Sensory Profile. There is a fillable Patient Intake Form available online. The Parent Checklists and Short Sensory Profile will need to be filled out and returned separately. They will be sent to clients via email. Paperwork can be returned by email, fax, or by mail.

STEP 3: Verification of insurance eligibility and benefits.

As a courtesy to our clients and families, Sensory KIDS will verify your insurance information and eligibility. We are in-network with Aetna, BCBS, Cigna, Moda, OHP (Care Oregon, OHSU, and Open Card), and Tri-Care West. We are out-of-network with Providence, Pacific Source, and UHC, but can still bill against out-of-network benefits, if your plan allows.

If a PCP referral or authorization is required, it will be notated in your verification document. Authorizations will be obtained by Sensory KIDS, but PCP referrals must be obtained by the client before services are rendered.

We require a PCP referral to be on file for all clients with an OHP plan.

STEP 4: Scheduling an initial evaluation appointment

Evaluations are scheduled in set morning times and run 60-90 minutes depending on insurance. Written reports will be available 2-3 weeks after the evaluation appointment. Comprehensive reports can be requested for an additional fee.

STEP 5: Scheduling parent meeting and treatment

Immediately following the evaluation appointment, a treatment email will be sent to establish a treatment schedule. Though we recommend each client remain with the evaluating therapist, our office will offer availability for all of our therapists on staff to better accommodate any scheduling restrictions. A round of OT is usually 30 sessions and we recommend scheduling treatment 2-3 times per week, if possible.

Once a treatment schedule has been chosen, we will follow up with offers to schedule the 50-minute initial parent education meeting with the treating therapist to review the evaluation results, discuss a treatment plan, and collaborate on goals.
