Getting Started at Sensory KIDS
The following steps apply to those who are seeking occupational therapy services only. Families interested in speech and language therapy or counseling should contact our office for further information on getting started.
STEP 1: Call our office at 503.575.9402, email us directly at firstname.lastname@example.org, or choose the “Inquire Today” bottom at the top-right hand of the page.
Our front staff will gather more information about the services you are looking for and begin the process by adding your family to our wait list.
STEP 2: Complete and return the intake paperwork
Provided in the links at the bottom of this page is our intake paperwork. You can also fill out intake forms online under the Intake Process tab. Our office will also send PDF versions of the paperwork via email upon your initial inquiry. Paperwork can be mailed out upon request.
The following forms will need to be completed and returned before an evaluation appointment can be offered:
- Intake Patient Form (either for children or for adults)
- Short Sensory Profile (not available online – will be sent via email)
All other policy forms and releases will be reviewed and signed at your family’s evaluation appointment.
STEP 3: Verify your family’s insurance eligibility and benefits
Once we have your insurance information on file, we will obtain a verification of your benefits and eligibility that will provide an estimate of coverage before services are rendered. If a PCP referral or authorization is required by your insurance plan, our office will work with your family to facilitate getting one.
STEP 4: Scheduling an initial evaluation appointment
We will do our best to accommodate your family’s scheduling needs. Evaluations normally take place Monday through Thursday mornings and may run 60 to 90 minutes. Evaluation appointments are scheduled at least a month in advance with the exception of cancellations that occur on our schedule. Written reports will be available 2-3 weeks after the evaluation appointment. Comprehensive written reports can be requested for an additional fee.
Requests for afternoon evaluations will result in a longer wait time as a result of our impacted afternoon schedule.
All evaluation offers will be made by email.
STEP 5: Scheduling parent meeting and/or treatment
We consider both your family’s availability and the availability of our therapists when scheduling treatment. A treatment email will be sent to you immediately following the initial evaluation appointment. Your child’s first session will be scheduled as a 50-minute adult-only parent meeting to review evaluation results, establish goals, and collaborate on a treatment plan. Parent meetings must be completed before therapy can begin. A round of OT is usually 30 sessions, and we recommend an intensive approach, with therapy occurring 2-3 times per week.
The processing of insurance authorizations may delay therapy start time.
